Short Term Investment Products (STIPs)

Welcome to the Short Term Investment Products (STIPs) information page. Here you will find information regarding the payout process as initiated by the Ministry of Finance. If you are already familiar with the process, you can access the forms here.

Which products are considered Short Term Investment Products?

These products include:

  • Executive Flexible Premium Annuity (EFPA), including EFPA II, EFPA IV and ESPA
  • Colonial Life Core Fund Series 6 (CSI)
  • Group Advanced Protection (GAP)
  • Guaranteed Annuity Advanced Performance Policy (GAAPP)

When will the Payment Process begin?


Applications will be processed in alphabetical order as listed below. Persons who are unable to make these appointed dates are kindly requested to visit the Processing Centres after February 29th, 2012.

The processing centres listed below will be open Monday to Friday from 8 a.m. to 2:30 p.m.

Documents which must accompany application forms:

  • Valid identification (ID Card, Passport, Driver's Permit - copy required)
  • Original contract (If lost, a Statutory Declaration of Lost Policy Form witnessed by a Commissioner of Affidavits)
  • Release of Assignment duly stamped by the Board of Inland Revenue (if applicable)
  • Notarized letter and ID for clients residing abroad
  • Power of Attorney (where applicable)
  • Deed of Assignment and Declaration of Trust Schedule I or Deed of Assignment and Declaration of Trust Schedule II - (Please print these forms on both sides on legal sized paper - 8.5 x 14 inches)
  • Signed Statutory Declaration Form (and witnessed by a Commissioner of Affidavits)
  • For Contracts where there are joint owners ("AND/OR", "AND"), all owners MUST sign
  • Official documentation with respect to Owner's Bank, Branch and Bank account number

Other important information:

  • Values payable were independently validated
  • For each contract $75,000 or more, a separate application form must be submitted
  • Monies that are to be paid to Contract Owners will be deposited directly into their Bank account
  • All foreign currency funds will be paid in TT dollar equivalent
  • Holders of the Colonial Life Core Fund Series 6 will be processed at a later date
  • The closing date for applications will be June 30th, 2012.

Download and print the forms that are relevant to you. All forms must be printed on legal sized paper - 8.5 x 14 inches (with the exception of the Statutory Declaration of Lost Policy form which must be printed on letter sized paper - 8.5 x 11 inches).

Form Purpose
Payment Request Application Form This form MUST be completed by all clients to access the Principal Balance on their Short Term Investment Products (STIPs). (Please print this form on legal sized paper - 8.5 x 14 inches).
Statutory Declaration of Lost Policy Form This form must be completed by clients who have lost their original STIPs contract and be witnessed by a Commissioner of Affidavits. The policy will be advertised in the press and final processing will begin six weeks thereafter.
Deed of Assignment Declaration of Trust - Schedule 1

This is to be completed by clients whose policies are NOT ASSIGNED. (Please print this form on legal sized paper - 8.5 x 14 inches).
Deed of Assignment Declaration of Trust - Schedule 2

This is to be completed if your policy is ASSIGNED to a bank or other institution. This form must be completed by BOTH Owner & Assignee. (Please print this form on legal sized paper - 8.5 x 14 inches).
Third Party Statutory Declaration Form - Schedule 5

This form must be completed by all clients to confirm that they are neither presently nor have been in 2008 - 2009 a Director or Officer of CLICO Investment Bank, Colonial Life Insurance Company [Trinidad] Limited, British American Insurance Company (Trinidad) Limited, Caribbean Money Market Brokers or CL Financial Limited. (See definition of 3rd Party here). The form must be witnessed by a Commissioner of Affidavits. (Please print this form on legal sized paper - 8.5 x 14 inches).
