Welcome to the Short Term Investment Products (STIPs) information page. Here you will find information regarding the payout process as initiated by the Ministry of Finance in relation to Credit Unions and Trade Unions Clients of Colonial Life Insurance Company (Trinidad) Limited.

Which products are considered Short Term Investment Products?

These products include:

  • Executive Flexible Premium Annuity (EFPA), including EFPA II, EFPA IV and ESPA
  • Colonial Life Core Fund Series 6 (CSI)
  • Group Advanced Protection (GAP)
  • Guaranteed Annuity Advanced Performance Policy (GAAPP)

When will the Payment Process begin?


Applications will be processed beginning September 26th, 2011 through to March 31st, 2012 at the following processing Centres.

Documents which must accompany application forms:

  • Valid Identification of authorized signatories/officers of the company
  • Original Contract or Certificate (If lost, a Declaration of Lost Policy Form or Mutual Funds Statutory Declaration as to loss of certificate witnessed by a Commissioner of Affidavit)
  • Release of Assignment duly stamped by the Board of Inland Revenue (if applicable)
  • Deed of Assignment and Declaration of Trust Form or Deed of Trust Schedule 1 or Schedule 11 (to be completed and signed at the Processing Centres)
  • Signed Acceptance of Government Payment Form Schedule 3 or Schedule 4 if policy is assigned
  • Contact Information (Email, Phone- mobile and/or business)
  • Signature of Authorized Signatories duly affixed with organization’s stamp
  • Current Board resolution of authorized signatories/officers of the company
  • An official document indicating the name of Credit Union or Trade Union Bank and Branch, as well as type of account and account number and account holder’s name (e.g. Copy of recent Bank Statement or letter from Bank)

Other important information:

  • Values payable were independently validated
  • For each contract a separate application form must be submitted
  • Monies that are to be paid to Contract Owners will be deposited directly into their Bank account
  • The closing date for applications will be March 31st, 2012

EFFA/GAP
(Credit Unions/Trade Unions Clients)


Form Purpose
Payment Request Application Form This form MUST be completed by eligible credit unions and trade unions to access the Principal Balance on their Short Term Investment Products (STIPs). (Please print this form on legal sized paper - 8.5 x 14 inches).
Statutory Declaration of Lost Policy Form This form must be completed by eligible credit unions and trade unions which have lost their original STIPs contract and must be witnessed by a Commissioner of Affidavits. The policy will be advertised in the press and final processing will begin six weeks thereafter. (Please print this form on legal sized paper - 8.5 x 14 inches).
Schedule 1: Deed of Assignment and Declaration of Trust - STIPs This form is to be completed in triplicate in the presence of a CLICO representative and is applicable to STIPs EFPA contracts. (Please print this form on legal-sized paper 8.5 x 14 inches).
Schedule 2: Prior Assignment - Institutional Investors - STIPs This form is applicable to EFPA contracts that are assigned and is to be completed in quadruplicate.. (Please print this form on both sides on legal sized paper - 8.5 x 14 inches).
Schedule 3: Acceptance of Government Payment - STIPs This form is applicable to EFPA contracts. (Please print this form on both sides on legal sized paper - 8.5 x 14 inches).
Schedule 4: Acceptance of Government Payment - Prior Assignment - STIPs This form is applicable to EFPA contracts that are assigned. (Please print this form on legal sized paper - 8.5 x 14 inches).



CSI-6
(Credit Unions/Trade Unions Clients)


Form Purpose
